How do I switch to Ascension Rx?

STEP ONE

Fill out a transfer form

Provide your contact details and medication allergies. Consent to text messaging for updates. After filling out the form, we’ll send you an email confirmation and call you to finalize your medication transfer.

STEP TWO

Talk to your doctor

Ask your doctor to send your prescriptions to:
  • Ascension Rx 1001 Home Delivery for maintenance medications
  • Ascension RX 1303 - Specialty for specialty medications
Your doctor can also fax prescriptions to: 1-833-347-0798.

STEP THREE

Add your payment details

After receiving a text message confirming your account setup is complete, add your payment details. This step is required for delivery. Your medications will then be delivered to your doorstep.

Who is eligible to use Ascension Rx?

Ascension Rx Home Delivery is available to members with maintenance and specialty medications. Not all medications (acute) are good candidates for mail order, including antibiotics and medications that require special handling, such as pain medications. 

In addition to home delivery, Ascension Rx has community pharmacy sites that cater to both acute and maintenance medication needs, though access may vary depending on location.
Residents of Arkansas and California are not currently eligible. If you’re in these states, please continue refilling your medications at your existing pharmacy.
