What is Pharmacy Medication Management?

Medication Management services provided by our team are focused on facilitating access to your medications as well as evaluating, coordinating, and recommending medication therapies specific to you and your needs. The Pharmacy team collaborates with providers, Ascension Rx Home Delivery Pharmacy, and Maxor to assist in providing information, resources, and support for you to achieve the best possible outcomes with your medications.

    • Pharmacists and technicians address comprehensive medication management needs and assist patients with complex medication regimes.
    • Promote quality of care by collaborating with providers and other healthcare team members.
    • Provide medication assistance resources. 
    • Available to all SmartHealth members regardless of if prescriptions are filled with Ascension pharmacy or use Ascension medical services.
  • Who is on the Pharmacy Team?

    The Medication Management Team is composed of licensed pharmacists and technicians. Each team member possesses a deep knowledge of not only medications, but also SmartHealth benefits to assist with any medication related issue.

    The Pharmacy Medication Management Team provides information, support, and education at no cost to you as part of the Ascension SmartHealth plan benefits.

    • Provide education, resources, and facilitates management of chronic conditions such as hypertension, diabetes, hypercholesterol, and many others.
    • Deliver patient specific medication recommendations and collaborate with providers to assist in the achievement of your healthcare goals.
    • Facilitate access to your medications by coordinating with Home Delivery and Maxor.

    We are focused on providing high quality information, assistance, and support to help you put money back in your pocket and time back in your day as you take control of your health.
