SmartHealth Well-being Program: Care for Yourself in 2021

Whether you're on the front lines or in a support role, you are important to our mission—providing personalized care that improves the health and well-being of our communities. For this important work, it's vital that you begin by caring for yourself. That's why Ascension continues to offer the SmartHealth Well-being Program for associates.
The 2021 program will launch on Jan. 2, 2021. Remember, if you sign up for an Ascension SmartHealth medical plan, you can earn $100 just for completing an online health assessment, and another $125 in Healthy Rewards by seeing an Ascension primary care doctor for your annual exam or well care visit. Like always, these rewards will be deposited into your HSA or HRA (depending on which medical plan you have) and the money can be used to pay for eligible healthcare expenses.
Important to Know
2020 Well-being Program
- The 2020 program ends on Dec. 31, 2020. Remember, we extended the deadline. You have until the end of the year to complete your well care visit* ($125 Healthy Rewards) and online health assessment ($100 Healthy Rewards).
- You will receive your Healthy Rewards based on when we’ve captured your activity completion:
- Before Nov. 30: Receive Healthy Rewards in your last December paycheck
- After Nov. 30: Receive Healthy Rewards in 2021
2021 Well-being Program
- 2021 program dates: Jan. 2 - Sept. 30, 2021
- Get started! Go to: join.virginpulse.com/smarthealthwellbeing and follow the instructions to set up your account.
- Healthy Rewards: Earn $225 in 2021.
- Receive $100 for completing the online health assessment
- Receive $125 for completing annual exam with Ascension primary care physician (Tier 1). Visits to physicians in the National Network (Tier 2) or Out-of-Network (Tier 3) are not rewardable. Visits must be completed by Sept. 30.
- Rewards awarded on a quarterly basis.
- Associates will still have access to all challenges, journeys, trackers and other wellness resources, although these are no longer rewardable activities.

*Annual PCP well visits provided by physicians in the Ascension Network (Tier 1) through virtual telemed services will count toward this wellness activity and earn Healthy Rewards. This includes well visits provided through Ascension Online Care. Telemed visits must be billed appropriately to be rewarded, so be sure your provider marks them as a well visit.
