Enhanced Disease Management Program for Diabetes

Ascension SmartHealth is pleased to offer an enhanced disease management (EDM) program to members and dependents with diabetes. The program works in partnership with your doctor to offer personalized and integrated care from a team of specialists, including care managers, pharmacists and behavioral health specialists.
By working together across all points of care, we can help you better self-manage your diabetes, while giving you the confidence you need to make informed choices regarding your health.
Led by Ascension’s care management team, the program can help you with:

Understanding and adhering to your medications;

Self-blood glucose monitoring;

Recognizing the signs of low and high blood glucose levels;

Nutrition coaching for carbohydrate counting and weight management;

Scheduling your recommended annual screening for diabetic complications;

Blood pressure and cholesterol management;

Optimizing your physical activity levels to meet recommended guidelines; and more.
The program is available at no extra cost and is included as part of your Ascension SmartHealth medical plan.
